To add a bit of context why admins use these approval features, it's not just the spam, there are abusers using abusive usernames which have made it through instant signups but not made it through manual approval.
As @lazyb0y points out though it might be worth testing if manual approval alone (without the question) is good enough to put them off.

might also be interesting to understand if with this procedure spammers generally dont even send the request for an acvount or if admins really read the why answer and decide by that if they let people join.the latter could actually add a lot of friction for "good" users as they would be afraid to get a no just because some random problem with the reason they give

That's a good point!
By the way, just came to mind that there's another problem with instant signups: if mods stop moderating, an unmoderated server with instant signups may fill with nasty people.
If there's some kind of approval switched on, it means there's a failsafe system where abandoned/semi-abandoned servers can't fill up with bad people (or good people).
