In the past he had absolutely no support for his problem and bringing it up would likely have resulted in *less* support rather than more (due to ostracisation). So he found a way for his problem to be constructive in the Maquis. On Voyager, that outlet was gone and he couldn't adapt. But then he was given another path and he embraced it.

He's not an evil person, he just has personality defects that need support.
