We had a guy who was hired to test changes to specific features, but instead of testing the feature branch he'd merge it into master and test the master branch.
He called it 'doing approvals'.
Didn't last long, obviously.

For me this one was more fun than frustration. Not so much for the other party though..
Our organisation has a number of certifications. For this we are regularly audited. Traditionally one or two people will visit us and we go through documentation, registrations, visit sites, have a nice lunch, talk to people etc etc.
In april 2020 Shit Happened though. So it was agreed to delay some physical site visits and do as much as we can via teams (note the lowercase and lack of MS).

Key information: our office is in the central part of the Netherlands. I'm based in Denmark, my coworker in the Dutch north, one auditor (from the certification company) ten min from to the office and the other, a 64yr old with a dislike of computers.. 250km away in Belgium.
So we agreed to meet up at 7:30 am. The main auditor send an agenda invite "7:30, with teams".
Guess which of the two auditors had left his bed really early to be in time for the physical meeting of our "audit team".

@folfdk Yup. Turns out at no point in the weeks discussing this had he understood "teams". It's just not on his radar.
(despite having it on his laptop, having received a box with a headset and camera from his employer etc etc)
Security footage shows him arriving at 7:25. Putting on his facemask and waiting until exactly 7:30 until ringing the bell.
Couple of minutes later a technician had handed him a coffee, we had talked on the phone, and he was off back home..

Communication is important in a lot of ways.
Never assume
(At my previous job at Novo Nordisk there would always be at least 3 different meanings of a three letter abbreviation. Often caused some confusion!)
