I find it hard to choose an instance. All I want is to share footage that's too long for Mastodon, occasionally.
The first dozen I looked at seem to be aiming at a niche I don't belong in, and they all have few followers, like 5 to 40 or so.
Is there a better approach?

There are some good instances listed in the guide at https://fedi.tips/how-to-publish-videos-and-audio-on-peertube and there are a couple of general interest ones listed.
The number of followers of other accounts doesn't really affect you at all, I don't think it's useful to pay attention to them. Your follower numbers are mainly to do with how many people discover your account on non-PeerTube platforms like Mastodon etc.

So if instance1 follows instance2 than all of the videos on instance2 show up on instance1 search. For instance1 videos to show up on instance2 search. Instance2 will have to follow instance1.
This make #PeerTube not function the same as #Mastodon and I see no point in using it because of that. -

Yeah it's really weird, the way it works. I can't tell if there's genuinely no content on Peertube or if the instance I signed up with isn't federated with instances that have content I'm interested in. -

You can find an official overall search system for PeerTube at https://sepiasearch.org
Some PeerTube instances also use this in their search boxes, others don't.
Admins can choose how much their instance federates, and how much of other instances are visible on their instance. The reason for this is that some nasty people created hate-filled instances and most admins don't want these hate videos ending up on their own instance.

Can I stream live to it and twitch at the same time?
Some people do that, you can stream to Twitch and PeerTube and OwnCast at once.
I've seen some streams where they've combined the chat displayed on the video stream, and you can see messages coming in from many different services.
