But I think I moved from one server to another without any problems.

I'm glad it went smoothly. You should find the vast majority of your followers will come back over the next few days. Minus, the non-active, dead, accounts. -

I think they might all be moved over, other than one which only joined Mastodon last month, sent a couple of random private posts, then something happened. I think they may have been suspended or something, but I don't know what a suspended account looks like.Anyway, it was very fast, matter of minutes rather than hours ... But it helps I only have 69 followers ... Probably something to do with the speed of the transfer I guess ... If I had the number of followers you have it would no doubt take a few days.

Glad it has all gone through smoothly, it's always a worry, no matter how many times you might end up doing it. But FediTips does provide a most excellent guide.
If you press on the account header of the one you are not sure of, it should come up with something like a 404 error, or even account suspended when you get to its home page. That let's you know if it was deleted or just naughty. -

An account named Melinda sent me a private post thanking me for following. Then 10 minutes later sent an identity private post. Seemed odd posting behaviour. If they did that on enough accounts I assumed it may have been flagged. Anyway, that was the only account which failed to move over. I haven't counted them individually but numbers add up.

If you see something suspicious it's worth reporting (click ... on suspicious post and "report"). There is no automated flagging system, everything is moderated by humans. Server admins rely on human beings reporting odd stuff.

One thing that did puzzle me:
I have my accounts set up to approve follow requests.
I saw follow requests from your new account for each of mine which, of course, I approved.
But I thought account migration bypassed that approval step. I guess not.

The account transfer bypasses requests for your followers (people who follow you), but not the people you follow (people you follow).
At a technical level, follows (people you follow) after transferring are treated as new follows, so they still require request confirmations.
The reason for this is the follower transfer includes a system for verifying you as owner of both accounts, but the follows transfer doesn't. The new request is to stop spammers.
