I used to be in private contracting. My counterpart on the federal side was a peevish career fed who always seemed to want to find scapegoats to blame. She said this overtly one time when we had some sort of slip-up on a contract task and I said, "I'd like to focus on finding solutions here," and she actually said with her mouth-hole, "no, I want to find out who's to blame!"

I later ended up working with her as a fed when I joined government.

Mistakes are how we learn, not just how we fuck things up. But it's important to discuss mistakes to avoid the latter just as much as to achieve the former.
